Important Business Update For All Clients
July, 16th 2026
Dear Clients,
I am writing to inform you that I have made the decision to close my private counseling practice after 19 years of management, supervision and ownership. As a result, our professional therapeutic relationship will formally conclude on 9-23-26 as that will be the last day of therapeutic sessions. The office doors will be permanently closed on 10-1-26.
Please be assured that this decision is entirely due to my own personal health and life transitions. It is not a reflection of you or our work together. It has been a true privilege to accompany you on your journey, and I want to applaud the dedication and hard work you have put into showing up, working hard, and being challenged in your personal growth. Thank you for allowing me to share this with you.
The priority during this transition is to ensure you experience no gap in your mental health care should you continue to pursue ongoing services. Please remember it may take time for you to feel comfortable in working with a new provider and it may take a few sessions.
Mental Health Emergency
If there is a mental health emergency that occurs during the transition please call 911, go to your local emergency room for a mental health assessment or call 988. If you or a loved one becomes suicidal, or are at risk of self-harm, or harming someone else call 911 immediately for assistance.
Important Information Regarding Your Medical Records:
Your clinical files are confidential and will remain securely stored in accordance with state and federal legal guidelines. After October 1, 2026, your records will be managed and maintained by Cariend a medical records custodian, to manage the records for the next 7 years. Cariend is not a medical provider and does not handle billing, medications, or referrals for the closed practice.
Requestors should be prepared to complete the Records Release Authorization Form and provide a copy of government-issued photo ID, as required by HIPAA. The fastest way to process requests for copies of medical records is to visit Cariend’s website and follow the instructions posted here: https://www.cariend.com/request-records/
If you do not have access to a computer or internet, Cariend can be reached by phone at 404-991-7080 – please note the HIPAA laws do not allow medical records to be released based on a phone or email request.
